Dr. Kelly Axsom: Driving better outcomes in heart failure

Cardiologist Dr. Kelly Axsom wants to help her heart failure patients live their best life for as long as they can. Inspired by patients battling heart disease, she spearheaded a team of multidisciplinary experts to launch an innovative program designed to monitor patients' cardiac health within the comfort of their own home. A central team remotely measures and monitors daily blood pressure and pulmonary artery pressure for patients across the New York Metropolitan area to proactively manage changes in patients data helping to prevent patients from coming to the hospital.
