There are two ways of doing a double eyelid procedure: an incisional approach and a non-incisional approach. In Dr. Prasad's practice, he basically makes that choice based on a couple of things: First if there is extra skin then the incisional procedure will be necessary, if there is extra fat or fat getting in the way of a well-defined crease, then an incisional procedure is necessary. A lot of relatively young thin males actually do not have fat and extra skin. When Dr. Prasad rolls up their skin a little bit with a Q-Tip, he can see that it would be probably be fine with just an infold with no removal of the skin. In those patients, he does a non-incisional procedure. Technically, a non-incisional procedure is still surgery but instead of making an incision and removing skin, he makes needle openings in various points in the upper eyelid, and then passes a stitch in a way that engages the skin and the underlying muscle. In this way, when the eyes are open, the skin folds in.
