Someday I'll get my own PS4, but for now here's a repair on my friend's console.
There isn't a lot of reinforcement on the top of, well, at least these Slim models (I have no experience with the original nor the Pro) and in this case, enough pressure was put on the top of the unit right where the optical drive is. The metal was bent just enough to have the disc grind on the edge.
Pretty easy fix, these are quite simple to work-on.

iFixit tutorial provides easy instructions on disassembly so don't ask me for that.
