The quadriceps are the most important muscle from the beginning phases of anterior cruciate ligament reconstruction (ACLR) rehab. The quadriceps is also the most difficult to engage because of tibiofemoral (knee) joint effusion and disuse. This video captures his first treatment visit, where he is six weeks out from surgery.

He is performing the straight leg raise in a seated position to maximize recruitment of vastus medialis oblique (VMO) compared to a supine position. To improve his VMO recruitment, a neuromuscular electrical stimulation (NMES) is placed on his right thigh (distal VMO and mid-vastus intermedius). He’s holding onto a strap that is looped around his foot and assisting (as little as needed) to maintain a straight leg - you do not want to perform this exercise with a quadriceps lag! The tempo of the exercise is one second for the concentric phase and four seconds for the eccentric phase.

He is wearing blood flow restriction (BFR) bands to improve his quadriceps strength and hypertrophy by creating an anabolic environment. BFR bands with low resistance and high-repetition exercises will maximize muscle fatigue without overloading the surgical repair and knee.

Always consult your physician before beginning any exercise program. STOP exercising if you experience any symptoms of weakness, unsteadiness, light-headedness/dizziness, chest pain or pressure, nausea, or shortness of breath.

Performing these exercises should not cause pain. If you experience pain while exercising, decrease the speed of your movement. If the pain persists - STOP. Mild soreness after exercising may be experienced after beginning a new exercise.
