1) Copy 2 of all your W-2s
2) IT-40 PNR (must)
3) Schedule A (must)
4) Schedule C (if filled)
5) Schedule D (must)
6) Schedule IN-DEP (if filled)
7) Schedule F (if filled)
8) Schedule H (must)
9) CT-40 PNR (if filled)
