The external rotation lag sign evaluates the rotator cuff and specifically infraspinatus and teres minor. The examiner passively flexes the elbow to 90° and positions it at the patient’s side. The examiner then moves the elbow into maximal external rotation and asks the patient to maintain that position. A positive test occurs when the patient is unable to hold the externally rotated position and the arm begins to drift into internal rotation. For teres minor or infraspinatus, sensitivity is up to 100%, specificity 98%!!
