Very interesting case of swelling on the right side of the face.
➡️A young girl reported to the OPD with a huge swelling on the face involving the Right cheek and the lip...Extending onto the orbital region.
It was a painless swelling which happened within a short period of a week.
✴️On intraoral examination , there was presence of vestibular obliteration on the right side of the free gingiva with respect to the 1st and 2nd premolar.
The 2nd premolar was partially erupted with mild pain on palaption.
🔷Hence, CBCT was taken as an Investigation Modality. CBCT showed a well defined radiolucency around the partially impacted 2nd Premolar with Maxillary sinus congestion.
💭The initial diagnosis for this clinical & radiographic presentation was a well defined "Periapical cyst" which was suspected of abscess formation leading to vestibular obliteration.
✅Treatment plan was:-
1. Flap reflection with releasing incision.
2. Exposure of the tooth root and direct visualization of the pattern of infection.
3. Pus drainage if present .
4. Trimming of the thin cortical bone above the cervical region of the 2nd premolar to expose the cystic lining.
5. Careful enucleation of the cystic lining from the sinus wall prevention perforation of the floor of the sinus.
6. Irrigation of the defect with saline and diluted betadine solution and trimming of the bony walls.
7. Closure of the flap with 3-0 absorbable suture(vicryl suture material).
8. Histopathological evaluation of the enucleated cyst.
9. Regular follow-up based on the H-P report.

Final Biopsy of the specimen was reported as "Odontogenic Keratocyst".
