Membrane fusion, one of the most fundamental processes in life, occurs when two separate lipid membranes merge into a single continuous bilayer. Fusion reactions share common features, but are catalyzed by diverse proteins.

Enfuvirtide is used along with other medications to treat human immunodeficiency virus (HIV) infection. Enfuvirtide is in a class of medications called HIV entry and fusion inhibitors. It works by decreasing the amount of HIV in the blood.

Enfuvirtide interferes with the entry of HIV-1 into cells by inhibiting fusion of viral and cellular membranes. Enfuvirtide binds to the first heptad-repeat (HR1) in the gp41subunit of the viral envelope glycoprotein and prevents the conformational changes required for the fusion of viral and cellular membranes.

The main mechanism of resistance to enfuvirtide is the selection of changes in a domain consisting of 10 amino acids, between residues 36 and 45 in the HR1 region of gp41.
