Doom 3: Resurrection of Evil (2005 / Original) done without taking any damage on the Nightmare difficulty and all enemies killed in 12 segments (1 for each level). This is the PC version.

Additional Notes:
How is it that you can jump off ladders without taking any fall damage?
In the expansion, you can safely fall from nearly every ladder, as fall damage has been disabled for most ladders throughout the game.

At 0:32:28, there's a body you can use to charge your artifact, just before the sewer section, which I missed during my run.

The Artifact Mechanic Explanation:
On regular difficulties, you upgrade the artifact gradually by defeating the 3 Hellhunters throughout the game. Each Hellhunter grants a new special ability for the artifact in the following order: Helltime (Slow Motion), Berserk (Dramatically increases weapon firepower), and Invulnerability (Makes you immune to enemy damage). In Nightmare difficulty, however, you begin the game with the artifact fully upgraded, meaning you don't have to worry about taking damage when using the artifact at the beginning.
