The game follows Emily Hartwood (Jodie Comer) and paranormal detective Edward Carnby (David Harbour) as they travel to Derceto Manor, a home for the mentally ill, to investigate the disappearance of Jeremy Hartwood, Emily's uncle.

The story, set in the 1930s in Louisiana in the southern United States, is experienced by the player from two different perspectives. Both Emily and Edward are playable in separate campaigns that differ significantly, each featuring their own cutscenes, locations and levels that do not appear in the other's playthrough.
