Once a fistula has formed wherein there is a focus of infection somewhere around the anal canal, usually in between the sphincters, there will be infective focus from where pus will be coming through different holes, or called as external openings. There may be one external openings, multiple openings, internal opening. In this stage what we call as a fistula, medical treatment has not much of a major role to cure the disease. So there are two approaches for treatment of fistula. One is there is Ayurvedic procedures , wherein they put a chemically treated thread through the tract that will slowly burn out and bring out. That is the conservative management but that is also a surgical procedure where under anesthesia you have to undergo that kind of procedure and it also requires long term observation and repeated treatment options are there that they have to be changing the thread frequently. So that is one way of doing the treatment of fistulas. The other is surgical treatment where we will use different gadgets and each will have different varieties of treatment . So there are multiple ways of treating fistulas. So the more number of procedures are there but we still don’t have an ideal procedure. So it has to be individualized, either we will cut open the fistula and allow it to dry wherever it is possible or we will put a drains into that and allow it to come put or we use lasers. Some radiofrequency and certain energy sources to clear the fistuals or we may be putting something called as situm, where a rubber band like thing will be kept for it to allow it to drain where it is not reachable for surgical procedures. Radical surgeries are also being done now where we will be totally be removing the whole fistulous tracts, go through the sphincters and repair the sphincters . So there are multiple different varieties of surgery. That is chosen depending on the need and depending on the situation for the individual patient.
