Understanding the Basics of LIT
In previous sessions, I’ve explained where LIT is indicated. Today, we focus on the pre-evaluation phase—understanding the immune reaction between the wife’s serum and the husband's lymphocytes. The donor mentioned in the report is the husband. The test checks whether the wife’s immune system already reacts to the husband's lymphocytes. If she has pre-existing antibodies against them, the test result will be positive.

This means the immune system recognizes the lymphocytes as foreign and mounts a defense. If that reaction is observed across dilutions, it indicates pre-sensitization—the maternal body has already seen and responded to these cells in the past. On the other hand, if the result is negative, it means there is no prior sensitization, and that’s where
LIT may help.

Why Sensitization Matters for Pregnancy
LIT aims to induce a protective blocking antibody. If the maternal system recognizes the paternal antigens early and develops this antibody, it may help the embryo implant and grow. Once these blocking antibodies are present, they act as a tolerance mechanism, protecting the fetus during early development. That’s the theory behind why LIT is sometimes effective in improving pregnancy outcomes.

The Scientific Limitation of LIT
Here's the challenge: the test checks the wife’s response to pure paternal lymphocytes, but a fetus is made up of both paternal and maternal antigens. The actual fetal antigen presentation is complex, controlled by HLA-C and uterine natural killer cells (uNK cells). Because we cannot precisely replicate this fetal antigen structure in the test, LIT only
works in about 10% of indicated cases.

That’s why LIT is recommended only in self-cycles—where both eggs and sperm are from the couple. It has no benefit in donor egg or donor sperm cycles, because you’re attempting to sensitize the immune system to cells that won’t be used in the embryo. This mismatch is the core reason why LIT doesn’t work universally.

Core Concept Behind LIT
The purpose of LIT is to create blocking antibodies that make the embryo appear familiar to the mother’s immune system. When this recognition happens, the immune system stops attacking the pregnancy. That’s the logic. But due to high antigenic polymorphism in fetal tissue, LIT remains unpredictable.

Should You Repeat the Test After LIT?
Yes, in most cases, we repeat the test 6 to 8 weeks after immunization to check for sensitization. If the antibody formation is still negative, another LIT cycle can be performed. Some protocols suggest repeating LIT every 6 weeks until the antibody response is established. But keep in mind, there’s no global consensus on this. For this reason, we still classify LIT as an experimental therapy
