On the 2nd of July 2021, the Lighthouse Trust celebrated 20 years of integrated service delivery, model interventions, operations research and capacity building in response to HIV in Malawi.
Lighthouse started in 1997, when staff of the medical ward and other departments of Kamuzu Central Hospital (KCH) - formerly Lilongwe Central Hospital, a tertiary hospital in Malawi’s central region, felt the urgent need to help patients and community members in the face of the HIV epidemic by providing HIV and AIDS care and support. Staff members who were off duty worked with community volunteers to begin home visits and offered HIV counselling, testing and treatment for opportunistic infections. Over time, the process became more organized and a major step in the institutional development process was the founding of the Lighthouse Trust in 2001.
Antiretroviral treatment (ART) started slowly in 2000 as a fee-for-service program financed by the Malawian government in a revolving fund, i.e. patient payments were used to buy new drugs. ART cost sharing continued until in June 2004 when ART became free-of-charge supported by the Global Fund.
Today, Lighthouse Trust is a WHO recognized Centre of Excellence (CoE) and is a Ministry of Health (MoH) certified training centre, which works in close coordination with MoH to operate four large integrated HIV testing, treatment and care clinics in Malawi: Lighthouse Center at KCH and Martin-Preuss Center at Bwaila Hospital in Lilongwe; Umodzi Family Center at Queen Elizabeth Central Hospital in Blantyre, and Tisungane Clinic at Zomba Central Hospital in Zomba. Lighthouse is currently collaborating with the MoH to upgrade Rainbow Clinic at Mzuzu Central Hospital to be the fifth HIV CoE.
With support from the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) through the U.S. Centers for Disease Control and Prevention (CDC), Lighthouse Trust provides support to over 57,000 people alive on treatment in the CoEs, and over 135,000 PLHIV in all the 56 facilities supported by Lighthouse nationwide. Lighthouse has also trained over 2000 individuals in both national curricular trainings and other Lighthouse Certified trainings.
Through HIV Testing, Clinical, Community Health, Training and Monitoring, Evaluation and Research services, Lighthouse contributes to and aligns efforts with the Government of Malawi’s implementation of goals, objectives, strategies, and action plans of the National Action Framework on HIV and AIDS. This framework focuses on reducing new HIV infections, improving the quality of treatment and care, mitigating the impacts of HIV and AIDS on individuals and households, and supporting systems needed to achieve these goals.
