STORYLINE:

A second separation procedure for 19-months-old conjoined Filipino twins began on Monday at Montefiore's Children's Hospital in New York.

Beginning at at 11:28 a-m local time (1628 GMT), it's the second and major procedure in the series of high-risk surgeries to separate Clarence and Carl Aguirre who are joined at the top of their heads.

Arlene Aguirre spent the morning kissing and playing with her sons before the surgery began, which may run as long as six hours, depending on how Carl and Clarence respond during surgery.

A team of 16 specialists, comprised of neurosurgeons, craniofacial surgeons, pediatric anesthesiologists, nurses and pediatric intensivists, performed a craniotomy; creating a second opening in the twins' skulls.

They also began to access important shared veins that must be tied off to enable the rerouting of blood flow.

Tissue expanders also were inserted under the skin covering each of the boys' skulls during surgery.

Instead of performing a single surgery, the medical team decided to separate the conjoined twins in as many as four procedures.

During last month's operation, balloons were placed underneath the boys' scalps to expand tissues.

The final operation is expected to take place before the end of the year.
