Most breast cancer patients have a common question on their minds: whether or not they require chemotherapy. If the doctor suggests that chemotherapy is needed then the next question is when? When the chemotherapy is provided prior to surgery it is called Neoadjuvant chemotherapy and if it is after the surgery it is referred to as adjuvant chemotherapy.

Giving chemotherapy prior to surgery is a form of treatment as well. The focus of this approach is fighting cancer by shrinking the size of the tumour, making it easier to extract and the surgery being minimally invasive. There are some basic requirements for which this approach is highly favourable. These are:

1. The cancer is locally advanced
2. The tumour is too big , i.e., more than 5 cms in size
3. The tumour involves the skin, underlying chest walls, etc.
4. The patient is desirous of breast conservation

If the biopsy of the affected region reveals a triple negative cancer, it means that the cancer is growing without any hormonal intervention or the tumour is advancing much faster. In such cases also it is seen that giving chemotherapy prior to surgery has had the best clinical outcomes. Above all, doing a chemo routine prior to surgery gives a good objective observation of whether the body is receptive to the treatment. This means that chances of breast conservation are also higher.
