A two-year-old boy from Iraq arrived at Medanta – Gurugram with a progressively enlarging mass in the left masticatory space. The tumour, unresponsive to chemotherapy, had caused severe oral dysfunction and significant weight loss. Imaging revealed a large, localised growth without metastasis.

At Medanta, a multidisciplinary team comprising paediatric surgery, head and neck oncology, plastic & reconstructive surgery, and paediatric oncology came together to plan definitive surgical management. The procedure involved tracheostomy, selective neck dissection, complete tumour excision with segmental mandibulectomy, and skull base clearance. Reconstruction was achieved using a right anterolateral thigh free flap with microvascular anastomosis—an especially rare and complex procedure in a child this young.

Postoperatively, the child required close monitoring in the Paediatric ICU, where fluid imbalances were successfully managed. Oral feeding was gradually restored, and decannulation was achieved by day 13. Histopathology confirmed a low-grade myofibroblastic sarcoma with clear margins and no metastasis, eliminating the need for further adjuvant therapy.

The patient was discharged in stable condition, tolerating oral intake, and placed under structured surveillance. This remarkable recovery underscores the power of coordinated multidisciplinary care in delivering both oncological control and functional rehabilitation in rare paediatric head and neck sarcomas.
