Dr. Corenman discusses general principles and treatment of spine disorders by implementing therapy tools for spine rehabilitation. He has previously spoken about how to diagnose a problem, how to read MRI and X-rays and know what tests to order (EMGs will be covered later), he answers the following important questions:
1. What do you actually do with these patients now that you have them?
2. How many tools do you have in your toolbox to take care of a patient?
3. How do you exactually use them?

For general rehab considerations:
-Work to strengthen and condition muscles
-Stretch ligaments, facts and discs
-Reset pelvic rotation to accommodate pathology
-Teach patients to adopt improved mechanical techniques (ergonomics) to prevent aggravating the disorder

Dr. Corenman dives into these high level issues: time and patient to understand a patient’s natural history, know your urgent indications for surgery, delayed symptom flair-up – the inflammatory cascade, hamstring stretch/reduce the lumbar flexion fulcrum, normal lumbo-pelvic rhythm, intentional reversal of normal lumbo-pelvic rhythm, bracing, cardiovascular training, initial neutral spine conditioning for the injured spine, core anatomy, spinal compression and distraction (opposing forces that occur with core compression), weak core muscles, engaging core muscles, lifting and Archimedes principal (ergonomics(, how degeneration predisposes to annual tears, multifidi force generation, avoid bacon, lettuce and tomato (BLT), lifting modifications (relearn lifting), and the neutral spine program.

He speaks about the pelvic station and relationship to lumbar disorders covering these topics: weight bearing percentages with different spine positions, pelvic station, pelvic flexion (anterior pelvic rotation), risks of pelvic flexion (anterior pelvic rotation), pelvic extension (posterior pelvic rotation), risks of flatting lumbar spine (posterior pelvic rotation), pelvic alignment and muscle distribution (“setting the sacral angle”), and learning posterior and anterior pelvic rotation.

Here are traditional therapies and contraindications: McKenzie extension exercises, Williams flexion exercises, incorrect therapy assumptions will increase symptoms, functional assessment, manipulation, superficial modalities for inflamed tissue, general modalities to work on inflamed tissue, medications, NSAID medications and potential complications, oral corticosteroids and complications, membrane stabilizers, opioids and narcotics, narcotic complications, tricyclic antidepressants, SSRIs, and muscle relaxants.

Dr. Corenman highlights specific disorders: Annual tears (LBA – Low Back Attack) which leads to acute annual treatment, Degenerative Disc Disease (DDD) and core strength, sports ergonomics of DDD, sitting positions and DDD, IDR, degeneration of the facets, facet disease, sacroiliac joint dysfunction, L4-S1 herniated discs causing root compression, herniated discs causing root compression II, ergonomics of foraminal stenosis and spinal stenosis in sports, piriformas syndrome, isthmic spondylolisthesis, degenerative spondylolisthesis, degenerative scoliosis, thoracic hyperkyphosis, lateral femoral cutaneous nerve pain (meralgia paresthetica), FAI hip and labral tears, greater trochanteric bursitis, and hamstring insertional tendonitis.
