Story in a nutshell (for those unfamiliar with the show):

Izzie has end stage cancer which has spread to her brain. It causes her to begin hallucinating her dead fiancee; to Izzie, his image means death. Feeling the end is close, Izzie begins to evaluate what's important in her life...her partner Alex and her close, but damaged, friendship with George.
