3 in each of Ancient Tomb, Skull Maze, & Crystal Cave.
4 in the Fire Temple & 1 in the Wizard's Tower.

Paying homage to Zelda, the secret rooms are hidden behind some cracked walls that can be cleared easily. The rewards aren't that great in the rooms, but you get a "feat" for finding them all.
