The Care Ecosystem: Telephone based Care Navigation for People with Dementia and their Car

This seminar will provide an overview of a model of care available to Medicare/Medicaid patients with dementia (PWD) and their caregivers through telephone as well as through other remote technologies to provide dementia care expertise, education, and support. Telephone and web-based technologies to support dementia patients and their caregivers at home, is one promising way to help ensure people have access to much-needed care, education, and support. Medicare has recently expanded services to include coverage for both people living with dementia and their caregivers who may have difficulty leaving home or lack adequate transportation to access the care and support they need. The Care Ecosystem is a model of dementia care designed to provide personalized, cost-efficient care to the growing population affected by dementia and their caregivers.
