Building a Sustainable Program of Evidence-Based Practice:
A Practical Guide for the Respiratory Care Professional

The purpose of this video presentation is to provide the rationale, importance, and some potential options behind why and how to develop a program of Evidence-Based Respiratory Care information into your practice by using quality-focused principles.
• Just a quick note, that while this presentation has a Respiratory Care emphasis, most of the information is applicable to all areas of healthcare delivery.

The United States healthcare industry is transforming from a “quantity” to a “quality” focus. Recently, numerous professional healthcare providers and organizations have advocated, and adopted Evidence-Based Practice (EBP) into healthcare delivery. Many of these organizations support the concept that activities supported by Evidence-Based Clinical Practice Guidelines should be prioritized to optimize healthcare delivery. Also, many healthcare organizations have encouraged their members to embrace and enact Implementation Science principles within their scope of practice.

Clinical Practice Guidelines are developed from evidence-based information and may also rely upon clinical expertise to support the guideline message. Clinical Practice Guidelines (CPGs) therefore provide the basis from which Evidence-Based Practice can be incorporated into healthcare delivery.

There is however not a direct pathway between Clinical Practice Guideline information and how to successfully implement that information into actual clinical practice.

This presentation will hopefully offer new information on potential quality constructs to consider when developing an Evidence-Based Practice program.

Implementation Science principles, and how they are intimately linked with healthcare quality programs, should enable the Respiratory Care change agent with developing processes that improve sustained delivery of quality care. Overviewed are some select quality-focused (Implementation Science) concepts, to consider, when developing, monitoring, amending, and validating an Evidence-Based Practice program. Ideally, this specific activity would use the American Association for Respiratory Care Clinical Practice Guidelines, or for the Pulmonary Diagnostician, the ATS Recommendations/Standards, as the source of procedure specific EBP information.

Implementation Science constructs reviewed include the
• Plan-Do-Study Act (PDSA) Cycle
• Plan-Implement-Evaluate (PIE) Cycle & the Getting To Outcomes (GTO™) 10 Steps™
• Lewin’s 3-Step Model
• Lean Six Sigma – Define, Measure, Analyze, Improve, and Control (DMAIC) Cycle
• Dynamic Sustainability Framework (DSF)
Other areas addressed include aspects of assessing Organizational Readiness (for change), use of Process Teams, along with some considerations during phases of Pre-Implementation, Implementation and when evaluating program Sustainability.

Now is the time for Respiratory Care professionals to follow their colleague’s example to improve quality, by adopting a focus on Evidence-Based Practice and applying Implementation Science constructs when developing new, or updating existing, healthcare programs.
