Musihimesama, the legendary bullet hell shoot'em up of CAVE.
Ver 1.5 (aka cave matsuri ver.) includes some revisions, bug fixes, new OST, and "Max" difficulty.
Literally, Max mode starts with max power, but you have only one bomb every life, kinda more enemies appear, and bullets increase and faster. so, Ultra Max is definitely the highest level of difficulty in this game.

In particular, the difficulty of true last boss increases greatly. Player can't skip phase 2 by pointblanking anymore. In addition, the final pattern is not dodgeable in ver1.5 because it's too fast compared to ver1.0, but it can still be beaten with techniques accompanied by suicide like ver1.0. it takes at least 2~3 lives to beat the final pattern in the case of M-Power (W-Power: 2, S-Power: 4)
