Information for New Entrant Safety Audit from DOT

1. Driver List
• Upload a list of drivers working for the carrier including first and last names, date of
birth, dates of hire, license numbers, and license states.
2. Vehicle List
• Upload a list of vehicles (tractors and trailers) owned by the carrier along with
associated unit numbers, VINs, and plates. Include an electronic logging device (ELD) or automatic onboard record device. Make, model, and year as applicable for each vehicle.

All carriers must have insurance sufficient to satisfy the minimum public liability.
