This presentation was given at the Health Quality & Safety Commission's event, patient deterioration New Zealand: current state and future developments, held on 29–30 November 2018.

The ADHB patient at risk (PaR) team was introduced to Auckland City Hospital in July 2017, alongside the implementation of the HQSC patient deterioration program. The NZEWS vital signs chart has been implemented in 100% of eligible wards, combined with the introduction of a structured responder system, which includes the PaR nurse specialists.
This presentation will describe:
1) The characteristics of the leadership and governance structure which have enabled change and organisational buy-in.
2) The implementation process used to introduce the afferent and efferent arms of the recognition and response system.
3) The barriers and enablers in the process.
4) Implementation of a database and some data from the first 12 months of the service.
5) Plans for the continued development of the team.
