From the lp album "Candlelight Melodies" (produced by the Longines Symphonette Society, 1967 version), Side B, Track #2.

Not sure where the Longines Society got the audio for this recording, but it most certainly is a live performance, as can be noted by the coughing and glasses clinking in the background.
