If you are an inpatient or in Homecare with St Patrick’s Mental Health Services (SPMHS), you can use Your Portal as a key part of your #treatment. It helps you communicate easily with your #multidisciplinary team (MDT), monitor your care plan, and complete forms sent by your MDT.

When you are being admitted, you will be asked to complete a Service User Expectations form. This outlines your expectations for your treatment and is used by your MDT to inform your care plan.

Your MDT will assign you a key worker. This person ensures you and your MDT work together on your care plan. To see who your key worker is, simply go to the ‘Events and Messages’ section of Your Portal.

Each week, your key worker will ask you to complete the Service User Views and Opinions form each week. You can do this on Your Portal.

You will also have a weekly key worker review meeting, where they show you your care plan and share updates with you. After this meeting, you can view your care plan in Your Portal. Just go to the ‘Treatments’ section and click ‘Plans’.
