Scoliosis is a structural spinal condition causing an unnatural sideways bend and twist within the spine. It is a progressive condition, meaning it will typically worsen over time. While some cases of infantile scoliosis may resolve on their own, it’s impossible to predict which cases will improve and which will worsen. As a result, scoliosis is generally considered progressive.
Most childhood scoliosis cases are diagnosed as idiopathic scoliosis, meaning the cause is unknown. Idiopathic scoliosis makes up about 80% of all scoliosis cases, and it is the most common type seen in children. Though the exact cause is not known, we do understand that growth triggers progression. This is why childhood scoliosis must be taken seriously, regardless of the age at diagnosis.
As scoliosis progresses, the curve becomes more severe, and both the size and rotation of the spine increase. This leads to uneven forces throughout the spine and body, resulting in more significant health effects. Progression complicates treatment because larger curves are more challenging to manage and reduce. Thus, the focus of treatment is on managing the curve, reducing its size, and preventing further complications.
Proactive treatment aims to prevent progression, whereas traditional treatments often wait until the curve worsens before responding. Once the curve becomes more severe, treatment becomes more difficult.
One type of scoliosis is congenital scoliosis, where patients are born with abnormal spinal development. Diagnosed at birth, this condition is considered a form of infantile scoliosis. Scoliosis diagnosed in patients between three and ten years old is classified as juvenile scoliosis. The earlier the diagnosis, whether infantile or juvenile, the more likely the curve will worsen because the patient has more time to grow.
The most common type of scoliosis is adolescent idiopathic scoliosis, which affects children between 10 and 18 years old. Rapid growth during this time is a key factor in progression, as the curve may grow along with the child.
Scoliosis is more common in females than males, with a four-to-one ratio. This may be due to females experiencing a shorter, more rapid growth spurt compared to males. Females typically undergo a growth spurt between ages 11 and 13, while males grow between 13 and 15. The earlier and faster growth in females may explain why they are more likely to develop scoliosis.
The goal in treating adolescent scoliosis is to manage the curve during this growth period until the patient is fully mature. Early detection and intervention are critical in limiting curve progression. The only type of scoliosis that might resolve on its own is infantile scoliosis diagnosed before age two. However, even in these cases, spontaneous resolution isn’t guaranteed.
Traditional treatments typically don’t address the condition until the curve becomes severe, often leading to invasive measures such as spinal fusion surgery. Before the advent of modern surgical techniques, conservative treatments aimed to manage scoliosis and prevent severe progression. While surgery became the dominant treatment, conservative approaches are now regaining popularity. Many patients are turning to proactive treatments such as chiropractic care, physical therapy, and exercises that enhance spinal flexibility and muscle support.
Scoliosis bracing has also improved significantly. Corrective bracing now helps both growing children and young adults. Since growth is the main trigger for progression, early treatment is essential. Waiting for scoliosis to resolve on its own can leave patients vulnerable to worsening curves, which are more difficult to treat. Therefore, it is recommended to seek treatment as soon as scoliosis is diagnosed to reduce the curve, as smaller curves are easier to manage.
