5) Original Copy of NBI Clearance of Applicant
issued within 6 months prior to the date of Application
6) 2 Valid Government Issued ID's
with picture of applicants responsible officers -clear copy
7) Latest General Information -Corporation/ DTI (Sole)
8)Personal Profile of Applicant, Responsible Officers
with 2 x 2 pictures on a photo paper
9) Company Profile with Pictures of Office and Warehouse
Premises with proper signage
10) Printed CPRS
profile of applicant and updated email notification with "STORED" CPRS profile
11) Previous Certificate of Accreditation
