These two forms, VA Form 21-4142 and 21-4142a, are crucial in the process of releasing your medical records to the VA. They can be found on the same PDF (linked below). It's important to note that if all of your medical records are VA records, then you do not need to fill out these forms. There are times when the VA requests these forms to be filled out, but this ends up being irrelevant since they already have the records. Further, it may be more efficient to manually submit the records yourself. The first form (VA Form 21-4142) is a general authorization form requesting your identification information on the first page. The second form, VA Form 21-4142a, is the actual release form. It is where the medical providers' information is presented for the VA to know who to reach out to for your records. This information includes the providers' names, what conditions were being treated, and the dates of treatment.
