Why Chemotherapy Is Needed Even After Surgery
A common concern patients express is, “If the tumor has already been surgically removed, why do I still need chemotherapy?”
The reason lies in how cancer spreads. Even after the most thorough surgical resection, microscopic cancer cells, called micrometastases, may still remain in the body. These cannot be seen during surgery or on scans but can cause recurrence in the future. That’s why adjuvant chemotherapy is critical, especially in Stage 2, 3, and 4 ovarian cancer. It is given after surgery to destroy these residual cancer cells.
In cases where the tumor is too large or located in a way that makes surgery unsafe, we use neoadjuvant chemotherapy, administered before surgery, to shrink the tumor and make it easier to remove. After 3 cycles, we reassess the tumor using scans and blood markers.
Which Drugs Are Used in Chemotherapy for Ovarian Cancer?
The most commonly prescribed drug combination in ovarian cancer chemotherapy is Paclitaxel and Carboplatin. This regimen has been the global standard of care for many years and is backed by extensive clinical trial data. It not only improves overall survival but also reduces the risk of recurrence.
What If the Patient Cannot Tolerate Standard Chemotherapy?
Not all patients are suitable for high-dose, three-weekly chemotherapy. Those with existing medical issues, such as heart disease, diabetes, or reduced kidney function, may not tolerate the standard regimen well.
Can We Add Targeted Therapy?
Yes. For eligible patients, we may include a targeted drug called Bevacizumab (brand name: Avastin). It blocks vascular endothelial growth factor (VEGF), which tumors need to develop new blood vessels.
Bevacizumab is particularly effective in cases with ascites, fluid accumulation in the abdomen. When combined with chemotherapy, it improves disease control and can increase response rates by 10–20%.
