Symptoms of IBS
• Abdominal pain or cramping
• Bloating and excess gas
• Diarrhea, constipation, or alternating both
• Mucus in stool
• Feeling of incomplete evacuation
• Symptoms worsened by stress or certain foods
Diagnosis of IBS
• Based on Rome IV Criteria: abdominal pain at least 1 day/week in the last 3 months, linked with stool changes.
• Clinical history and physical examination
• Exclusion of other conditions (e.g., IBD, infections, celiac disease)
• Tests may include stool exam, blood test, colonoscopy (if needed)
Role of Antidepressants in IBS Treatment
• Gut-Brain Axis Regulation: IBS involves abnormal communication between gut and brain; antidepressants restore balance.
• Pain Relief: TCAs and SSRIs reduce abdominal pain and discomfort via central nervous system pathways.
• Improved Bowel Function:
o SSRIs (e.g., Escitalopram, Fluoxetine, Sertraline) improve constipation-predominant IBS.
o TCAs (e.g., Amitriptyline, Nortriptyline) reduce diarrhea and urgency.
SNRIs (like Duloxetine, Venlafaxine) are not the first-line drugs for Irritable Bowel Syndrome (IBS), but they are sometimes used when IBS is strongly associated with chronic pain, anxiety, or depression.
How they help in IBS:
1. Pain modulation – By increasing norepinephrine, SNRIs reduce central sensitization and visceral hypersensitivity (common in IBS).
2. Mood improvement – IBS often co-exists with anxiety and depression; SNRIs treat both.
3. Gut motility effects – Serotonin reuptake modulation can indirectly help regulate bowel movement.
They act on both serotonin and norepinephrine pathways, which regulate gut–brain axis signaling, pain perception, and mood.
