Module I: An Introduction to Making Your Healthcare Wishes Known
Sets the foundation for practitioners to engage in the advance care planning conversations. Participants will become familiar with the seminal US Supreme Court cases of Nancy Cruzan and the Patient Self-Determination Act in addition to mastering the vocabulary of advance care planning. The training will focus on the importance of soliciting personal values and beliefs and when appropriate, identifying goals of care.
Module II: Conducting Conversations
Focuses on the practical application of ethical principles utilizing communication tools to create an experiential learning environment. Reviews the basic ethical principles of autonomy, beneficence, non-maleficence and just and what they look like in the context of advance care planning discussion.
Module III: Transportable Physician Order fro Patient Preferences (TPOPP)
Transportable Physician Orders for Patient Preferences (TPOPP is the next step in advance care planning for patients who live with an advanced chronic illness or have been told they have a limited life expectancy. The use of the TPOPP form allows the values and preferences of a patient to be married with current medical information as a means to identify treatment goals.
