Procedure Description:

The Brazilian butt lift ( BBL) is a procedure that consists of liposuction to areas of excess fat, usually in the abdomen and back with a subsequent fat transfer to the buttocks. The procedure is done under general and tumescent anesthesia which is mixed as follows : Normal Saline (Water and salt)+ lidocaine (Numbing medicine) + epinephrine (adrenaline). The procedure can also be done under tumescent Anesthesia and oral or intravenous sedation. The excess fat is aspirated using very tiny cannulas which usually measure only 4 mm in diameter. Body sculpting is performed this way. High definition Liposculpture outlining the underlying muscles can also be done in qualified patients if desired.. The aspirated fat is collected in a sterile container which is connected to the cannula. The fat is then Transferred to the Subcutaneous space between the skin and the muscle of the buttocks being careful not to inject into the muscle. Blunt 4 mm cannulas are used to safely re inject the fat and avoid perforating the muscle. The Wells Johnson High-Volume Precision system makes the procedure safer as it allows the surgeon to inject the fat at very low pressures in a completely closed sterile system. The Carboxy Pneumo Dissection consists of injecting carbon dioxide (C02) and applying a suction vacuum device in order to expand the subcutaneous space so that more fat can be injected safely away from the gluteal muscles. The carbon dioxide (CO2) attracts local oxygen to the tissues. The recently transplanted fat cells are now in an oxygen rich area which is ideal for their survival. Ultrasound guided gluteal fat transfer (UGFT) assures placement of the fat graft in the subcutaneous space ( under the skin layer), away from the muscle minimizing the risks of injection into the Gluteal muscle veins which can cause a fat embolism and death. Platelet rich plasma is obtained from a small amount of blood drawn from a vein usually in your arms. The blood undergoes centrifugation (spinning ) for three minutes in order to separate the blood clot from the serum. This platelet rich plasma has 20 times the amount of platelets, it has growth factors and mother cells. The platelet rich plasma connects the fat cell to its nutrient blood vessel. Once the fat cell is connected to the blood vessel it survives. The survival of the entire fat graft is improved. The pluripotent mother cells also become fat cells. Laser assisted lipolysis is also done once all of the fat has been harvested in order to kill any resistant fat that was not aspirated with liposuction. The laser also assists in tightening the skin, less bruising, less bleeding and a faster recovery. In cases of significant weight loss where the buttocks are sagging an open buttock lift can be performed at the same time as the fat transfer to the buttocks.
