Hello dear patients. Today we are going to review the case of skin cyst removal on penis. It is noted that quite often cysts appear on the area of frenulum of prepuce. This is possibly related to tension, to load on this part of the skin, particularly during sexual contacts, with the fact that excretory ducts of sebaceous and sweat glands may be obstructed under the influence of different mechanical factors, by different substances including lubricants, sperm etc. So, that’s why we have often seen the cysts that you can see on the picture, on foreskin, particularly in the area of frenulum. What to do in this case? Generally speaking, during such a picture, histological study is not required because this can be a sebaceous gland cyst, which is more common or a sweat gland cyst which is less common. This neoplasm is a subcutaneous blister which contains fluid. Often, the patients do not like it externally, they are scared of it and if the density is quite high, then it can mechanically obstruct during sexual contacts. So far I have removed about 15 of such neoplasms. Generally speaking, in such cases, external anesthesia which is usually done by cream, is not enough, so we do injection, usually of ultracain or lidocain intradermally.
This is usually not painful and preliminary anesthesia by emla cream can be done or by 10 percent solution of lidocain and then to do injection. Usually, one injection is required and an insulin syringe is used for it and after that a superficial removal is done. A small erosion develops. usually, a radiowave method can be used for removal because the skin on foreskin is quite thin and the use of laser or electrocoagulation can facilitate the development of scar and it would be quite uncomfortable for the patient. Generally speaking, it is enough to cut away the surface, the covering of this blister, to burn the base of blister. This doesn’t look scary, you can see it on the picture. Usually, during a competent superficial removal and if the doctor has enough experience, then scars do not develop because this is an intradermal neoplasm if the doctor does not get deep and does not actively burn the floor. Secondly, usually there are no relapses after such removal, again, if the doctor has experience at least I haven’t seen the relapses of such neoplasms in my practice. If there’s a scientific interest about what it is a sebaceous gland or a sweat gland, then a histological study can be done.
However, during such picture we can be sure that this is a cyst of the gland and not something else. So, in the majority of such cases, histological studies are not required. Usually, some patients only observe such neoplasms – we have such patients coming for examination and we have not seen any cases of these neoplasms getting malignant. The only thing is that once I have seen the case of repeated infection of such neoplasm after trauma that is, it can get purulent and being a complication, it can obstruct during sexual contact if this blister is overfilled and quite dense, so it can induce tenderness and unpleasant sensations during sexual contacts.
