The Annual Confidential Report (ACR) is a critical tool used by the Government of Sindh to evaluate the performance of its officers. Proper completion of the ACR form ensures an accurate assessment and contributes to the professional development of government employees.

Instructions for Completing the ACR Form:

Timely Submission:

Part I of the Performance Evaluation Report (PER) should be completed and dispatched to the reporting officer no later than January 15th each year.
The reporting officer is expected to forward the report to the countersigning officer within two weeks after providing their assessment in Parts II and IV.
Countersigning officers should finalize their comments in Part V within two weeks of receiving the PER. If a second countersigning officer is involved, they should also complete their assessment within a two-week period.
Accurate Identification:

Clearly write the names and designations of both the reporting and countersigning officers.
Ensure that all comments are legible and presented in the prescribed format to facilitate easy scanning and record-keeping.
Objective Assessment:

The reporting officer's evaluation should be specific to the job and reflect the work performed by the officer during the reporting period.
Avoid biased or evasive assessments, as the countersigning officer will comment on the quality of the evaluation provided.
Support assessments in Part III with comments against each relevant section.
Communication of Adverse Remarks:

Countersigning officers should underline in red ink any remarks they consider adverse and ensure these are communicated to the officer being reported upon.
All adverse remarks, whether remediable or not, should be communicated to the concerned officer, with a copy of the communication placed in the Confidential Report (CR) dossier.
Reporting officers should provide counseling to the officer under report when adverse remarks are recorded.
Consistency and Clarity:

Ensure that reports are consistent with the pen picture, overall grading, and comparative grading.
Comments should be clear, direct, objective, and unambiguous.
Avoid vague impressions based on inadequate knowledge or isolated incidents.
