Topic: Assess and Record Vital Signs Accurately

इस लेक्चर में हम यूनिट 6 – नर्सिंग फाउंडेशन के अंतर्गत रोगी के महत्वपूर्ण लक्षणों (Vital Signs) का सही मूल्यांकन और रिकॉर्डिंग करना सीखेंगे। यह नर्सिंग की एक अत्यंत महत्वपूर्ण कौशल है, क्योंकि इससे रोगी की शारीरिक स्थिति का पता चलता है और समय पर उपचार संभव होता है।

🔹 What are Vital Signs?

English: Vital signs are measurable clinical indicators that reflect essential body functions.

Hindi: महत्वपूर्ण लक्षण ऐसे नैदानिक संकेत हैं जो शरीर की मूलभूत क्रियाओं को दर्शाते हैं।

The four main vital signs are:

Temperature (तापमान)

Pulse (नाड़ी)

Respiration (श्वसन)

Blood Pressure (रक्तचाप)

In some cases, Pain & Oxygen Saturation are also considered as additional vital signs.

🔹 Importance of Vital Signs

English:

Help in early detection of health problems

Provide baseline data for treatment decisions

Monitor patient’s progress and response to therapy

स्वास्थ्य समस्याओं का प्रारंभिक पता लगाने में सहायक

उपचार निर्णयों के लिए आधारभूत जानकारी प्रदान करते हैं

रोगी की प्रगति और उपचार की प्रतिक्रिया की निगरानी में उपयोगी

🔹 Accurate Assessment of Vital Signs

Temperature: Use thermometer correctly (oral, axillary, tympanic).

Pulse: Count rate, rhythm, and volume at radial or apical site.

Respiration: Observe chest movements, rate, depth, and rhythm.

Blood Pressure: Measure using sphygmomanometer, ensure proper cuff size and positioning.

👉 तापमान, नाड़ी, श्वसन और रक्तचाप को सही उपकरण और तकनीक से नापना आवश्यक है।

🔹 Recording of Vital Signs

English: Document findings immediately, accurately, and clearly in patient records or charts.

Hindi: प्राप्त आंकड़ों को तुरंत, सही और स्पष्ट रूप से रोगी की रिपोर्ट या चार्ट में दर्ज करें।
