Middle ear infections can be overdiagnosed in autistic children, partly because examining the ears can be more difficult and distressing. A reliable ear exam requires the child to remain still and tolerate the otoscope, which can be challenging for children with sensory sensitivities. Incomplete or rushed examinations may lead clinicians to rely on indirect signs, increasing the risk of misinterpreting a red or poorly visualised eardrum as bacterial infection. Communication differences also play a role. Autistic children may not express pain in typical ways, may struggle to localise or describe discomfort, or may not report symptoms at all. Changes in behaviour, sensitivity to sound, or language regression can be mistakenly attributed to ear pain when they may reflect sensory processing differences or responses to environmental noise rather than infection. This overlap highlights the importance of careful assessment, repeat examinations when needed, and avoiding potentially unnecessary procedures although this can be practically challenging.
