"Why did Transocean's blowout preventer - the key failsafe mechanism - fail to shut in the well and release the rig? Through our investigation, we hope to learn more about what happened and what was done in the hours before the explosion. Apart from looking at the causes of the explosion, we are also examining why the blowout preventer - the BOP, as it's called - did not work as the ultimate failsafe to seal the well and prevent an oil spill."

"The one thing we do know is that on the evening of April 20th, there was a sudden, catastrophic failure of the cement, the casing or both. Without a failure of one of those elements, the explosion could not have occurred."

"We should not be making a rush to judgement. However, two things can be said with some certainty: The casing shoe was cemented some 20 hours prior to the tragic incident and had the BOP (blowout preventer) functioned as expected, this catastrophe may well not have occurred. Halliburton's confident that the cementing work on the Mississippi Canyon 252 well was completed in accordance with the requirements of the well owner's well construction plan."

STORYLINE:
The blame game was playing out on Capitol Hill on Tuesday as lawmakers questioned the executives of three companies about what caused the Gulf of Mexico oil disaster.
In testimony before the Senate Energy and Natural Resources Committee, the Chairman of BP America told Congress its massive Gulf oil spill was caused by the failure of a key safety device made by another company.
In turn, that company said BP was in charge, and that a third company that poured concrete to plug the exploratory well didn't do it right.
The third company, which was plugging the well in anticipation of future production, says it was only following BP's plan.
The finger pointing came out in the open on Tuesday as the Senate committee opened the first hearings into the spill that has been contaminating water in the Gulf of Mexico for three weeks and threatens sensitive marshes and marine life from Louisiana to Florida.
But they spent little time before trying to shift responsibility for the environmental crisis to each other.
