A kidney biopsy is a procedure where a tiny sample of kidney tissue is removed and examined under a microscope. For example, if a patient has long-term high BP and their ultrasound shows reduced kidney size, doctors may safely assume the cause without a biopsy. Likewise, if blood sugar consistently ranges from 200–400, HbA1c is above 7.5, and the kidney size appears normal, diabetes is most likely responsible, making biopsy avoidable.

However, when the underlying cause is uncertain or doesn’t follow typical patterns, a biopsy becomes essential. For example, a young adult with normal kidney size, no diabetes or BP history, but declining function, may be dealing with an immune-related or genetic issue. In such cases, initial tests like ANA or VMA are conducted. If these tests don’t reveal the cause, a biopsy helps uncover hidden conditions and allows doctors to begin targeted treatment.
