SILENCE IS DEADLY. If a person is choking and stops coughing, you have seconds to act. Here is the correct medical protocol for the Heimlich Maneuver (Abdominal Thrusts).

THE PROTOCOL:

Identify: Look for the "Universal Choking Sign" (hands at throat). If they can cough or speak, let them clear it. If they are silent, ACT.
Stance: Stand behind them with a wide stance for stability.
Landmark: Place your fist just above the navel (umbilicus) but below the breastbone (xiphoid process).
Action: Thrust IN and UP (J-motion). You are trying to use their diaphragm to push air out of the lungs to pop the object loose.
🩺 FOR MBBS STUDENTS / CLINICAL PEARLS:

Anatomy: Ensure you are well below the xiphoid process to avoid fracturing the sternum or ribs, which can puncture the liver or spleen.
Physiology: You are artificially increasing intrathoracic pressure to create an "artificial cough."
Red Flag: If the patient becomes unconscious, STOP thrusts immediately. Support them to the floor and begin CPR (Chest Compressions). The muscles relax upon unconsciousness, and compressions are effective at dislodging obstructions.
