Best strat is to dodge early rather than late in this fight. Sloths attacks look like they'll delay but he unleashes them immediately.

My Loadout for beating Sloth Level 22: HP Lvl 17 - Phys Lvl 1- Arcane Lvl 6 Barbs +4: Angelic Chaos +2 Chains +5: Demonic Shade +3 Lance: Balanced Juggernaut I made the Balanced Juggernaut enchancement just before fighting Sloth after finding it in the dark subway area just before the Vulgrim Plinth here in this area. Crawl under the subway train and avoid the two giant bugs their and collect the items in the room. The Juggernaut enhancement is the furthest away when you enter the room. In order to make any balanced enhancement you'll need to defeat a Chosen. The one that's available at this point of the game is found in the catacombs, just beyond the first axe wielding flame giant you face in a secret room. Defeat the enemy and claim your Chosen material. None of these items require grinding, just some fun exploration. Most of the levels and artifacts I gathered came from the use of itemized lurchers which can easily be attained in the world. This build is great for many of the earlier bosses who require you to dodge a lot so you can retaliate with a ton of arcane damage and have a generous amount of survivability for any mistakes you'd make.
