Six days after surgery, we did Aryana’s first clean out and removed her cast. She was in tears before she even saw her nose because she finally felt what it was like to breathe through her nose after six years of being unable to do so. Aryana underwent primary rhinoplasty at another practice six years ago and was told after the procedure everything was fine, and that her breathing issues were likely allergies or something else. Having essentially no sense of smell, no nasal airway, Aryana was breathing through her mouth, and all the things that go along with an inability to breathe through one’s nose. Often, when patients undergo primary rhinoplasty and then have breathing issues afterwards, they’re sometimes told by their surgeons that it’s normal or that there’s nothing wrong. To some degree, that may be true with swelling in the early phase, but years after rhinoplasty, if you’re still having issues with breathing, then it might require another opinion.

When I started her #RevisionRhinoplasty procedure, I was shocked. I saw that grafts had been placed in inappropriate places, and eroding through her skin. Her septum was so buckled and crooked that it was 100% blocking her airway. As a revision rhinoplasty specialist, I actually get excited by the opportunity to correct a nose that has a lot wrong with it. I can never guarantee a perfect result or a perfect airway, but I always itemize the most important things for each patient that we really need to get right. Airway is always at the top of the list for those suffering from breathing issues because their quality of life has been affected, and often there are some cosmetic concerns that go along with it.

For Aryana, airway was the most important thing, but her nasal tip was drooping and crooked. I used a rib graft to reconstruct her nose and rebuild her entire tip and nostrils because they collapsed with inspiration. I replaced the entire front part of her cartilage because it was so crooked, it was unusable. We call this an anterior septal reconstruction. Aryana’s surgery was very complex. It took, I believe, nearly 5 hours to perform, but everything went really, really well.
