Here is simple guide for OFWs arriving at CLark with the latest quarantine protocol. This is based on my personal experience with the new 10 day mandatory Hotel quarantine for all arriving passengers.

Before landing to Clark Airport, the usual forms to be filled up will be distributed while on board.
Health declaration
Customs declaration
Immigration clearance/Arrival card

Upon arrival, all arriving passengers will be gathered in one hall and few minutes of orientation will be given and certain forms will be distributed for filling up. OWWA counter will then assign your designated hotel.

Remember, the address that you will put on all the forms should be your FINAL destination after quarantine. That would be the basis of your Hotel assignment, OWWA transportation and Quarantine Certificate.

Documents needed upon swabbing day.
Passport
QR CODE for the E-CIF registration from Red Cross
Case Investigation form (CIF) given at the airport. You will be given by this form at the arrival hall that you need to fill up and keep until the schedule of swab test.


Swab Test will be done at a designated room inside your quarantine hotel. You need to wear facemask and faceshield on swabbing day.

My swab Test result was released in less than 24 hrs. I received an email from Red Cross the following day morning.
