First generation biomarkers are really confusing due to their inconsistency, even within single-drug programs, where they work in the second-line setting but not the frontline, Powles explains.

Powles says there is an obligation to understand these. A frontline trial can clarify this over the next year or so. Trials coming out will demonstrate the role of these frontline biomarkers, so Powles adds that PD-L1 should not be thrown out yet.

One year ago, everyone assumed second-generation biomarkers would supersede the first-generation, but Powles no longer believes that is the case. He adds that tumor mutational burden and specific signatures like epithelial-mesenchymal transition and TGF-β1 may herald sensitivity and resistance. These may facilitate the building of a biomarker platform and increase confidence that physicians are selecting the right treatments for patients.
