Cardura Doxazosin uses
1. Overview
Generic name: Doxazosin
Drug class: Alpha-1 adrenergic blocker

2. Uses
Approved uses
1. Hypertension (HTN)
o Reduces blood pressure by decreasing peripheral vascular resistance.
o Usually not first-line; often used when other agents are unsuitable or as add-on therapy.
2. Benign Prostatic Hyperplasia (BPH)
o Relieves lower urinary tract symptoms (e.g., weak stream, hesitancy, nocturia) by relaxing bladder neck and prostate smooth muscle.
Off-label uses
• Ureteral stone passage facilitation (as part of α-blocker therapy)
• Raynaud’s phenomenon (rarely)
• Congestive heart failure (historically, but now discouraged due to trial data showing increased risk of heart failure)

3. Side Effects
Doxazosin effects are mostly due to vascular smooth muscle relaxation and systemic vasodilation.
Common side effects
• Postural (orthostatic) hypotension especially after first dose (first-dose phenomenon”)
• Dizziness, lightheadedness
• Fatigue, drowsiness
• Headache
• Edema (peripheral swelling)
• Nasal congestion
Less common but important
• Palpitations, reflex tachycardia
• Syncope (especially on initiation or rapid dose escalation)
• GI symptoms (nausea, diarrhea)
• Shortness of breath (if fluid retention occurs)
Rare but serious
• Priapism (persistent, painful erection) urological emergency
• Severe hypotension leading to falls or ischemia
• Hepatic dysfunction (rare)


5. Contraindications
Absolute contraindications
• History of hypersensitivity to doxazosin or other quinazoline derivatives
• History of orthostatic hypotension with severe symptoms (relative in some cases, but often avoided)
Relative/precautionary contraindications
• Hepatic impairment metabolised by liver; dose adjustment or avoidance may be necessary
• Severe heart failure not recommended due to increased risk of worsening heart failure in the ALLHAT trial
• History of severe hypotensive reactions
• Cataract surgery planned risk of Intraoperative Floppy Iris Syndrome (IFIS); ophthalmologist must be informed before surgery

6. Key Clinical Notes
• First-dose phenomenon:
o Marked drop in blood pressure after the first dose (or after restarting therapy) → risk of fainting
o Usually give first dose at bedtime to minimize symptoms
• Titration: Start low, increase gradually to reduce orthostatic hypotension risk
• BPH benefit: Works quickly (within days) for urinary flow improvement
• BP effect: Full antihypertensive effect may take weeks
