The Secret Service said on Friday that "complacency" led to a gap in protection allowing a would-be assassin to fire shots at former president Donald Trump at a rally in Butler, Pa. on July 13.

"Line of sight issues were acknowledged, but not properly mitigated. Issues were encountered the day of the visit with respect to line of sight concerns, but they were not escalated to supervisors. While some members of the advance team were very diligent, there was complacency on the part of others that led to a breach of security protocols," Secret Service Director Ronald Rowe said adding "These employees will be held accountable."

Trump was injured when bullets grazed his ear at a rally in Butler on July 13 when would-be assassin Thomas Matthew Crooks opened fire on Trump before being killed by return fire by the Secret Service.

The agency was providing an update on the investigations into the assassination attempts on Trump including on Sept. 15 when an individual was arrested after allegedly pointing a gun with a scope towards Trump's golf course at Mar-a-Lago while the former president was playing a round.

Trump was uninjured when Ryan Routh, 58, was spotted lying in wait at the perimetre of his golf course earlier this month. A Secret Service agent saw Routh and opened fire before Routh retreated to a nearby vehicle.

Rowe said Friday of the Secret Service's golf course protection of Trump that "the bottom line is the individual was detected."
