After years of focusing on morbidity at intubation, many clinicians and professional expert groups are concentrating on extubation issues. In this case, a man in the ICU has been weaned from the ventilator and is ready for extubation. The care team is concerned that he might now be able to maintain himself after extubation, and an airway exchange catheter(AEC) is placed (1). He is extubated leaving the catheter in situ. After about 15min he tires and can not maintain his O2 saturation. A tracheal tube is replaced over the catheter.
