The physician cannot enforce patient responsibility. That has to be internally in the patient. They have to be first, properly educated of what their problem is, and then logically after they've been told what are the different avenues of treatment, then logically they should decide. The biggest problem, or one of the big problems I see the patients make, and I understand it, is the delay in care. They delay their care to the point where their pathology becomes untreated disease process. Or what I call neglected disease, and neglected disease is a worse disease than what they started with.

And the barriers that occur for the treatment of that neglected disease, it can be either physical, can be emotional, can be financial, can be family, can be job. There's a lot of different hurdles that people have to overcome in order to make appropriate decisions.
