PRC Davao is located at Gaisano Mall of Davao, JP Laurel Street, Bajada, Davao City.

Their office is located at the 5th floor primarily the OSSCO ( One Stop Service Center for OFW'S). You can also trabsact at the different government agencies like NBI, Philhealth, SSS,Pag-ibig, CHED, DMW, and many more.

Proper dress code is needed. You cannot go inside if you are wearing shorts, sleeveless, skirt, slippers, tattered/ripped jeans.

Requirement for PRC ID renewal:
1.APPOINTMENT SLOT AT PRC ( You can get an appointment slot by signing into your LERIS account, Select transaction, Select Renew, Then type in your prc i.d. and profession. Payment is done through online at P450 pesos and you will also choose which PRC OFFICE you would like to transact.For me the nearest is PRC Gaisano Mall of Davao.

2.. PRC RENEWAL FORM (You can print this by signing into your LERIS account). Take note that your picture should be new and not the same as your previous picture on your I.D. Should be taken within 6 months.

3. CPDAS COPY OF CPD UNITS ( I printed the copy of my CPD UNITS in my CPDAS account by taking a screenshot. They need this to verify if you really obtained the needed CPD UNITS. For Nurses : 15 CPD UNITS. You can still renew your license even though you do not have CPD UNITS, you just need to sign on the undertaking form that you will provide the lacking units on your next renewal.This is accepted until December 2024 this year.

4. COPY OF SEMINARS/TRAININGS WITH CPD UNITS. ( They need these for attachment purposes the trainings/seminars where you obtained your CPD).

No need to photocopy your previous prc i.d. they will not be needing this anyway.
