Look at the top left in the background, my Durge and Astarion who got knocked down. It looks like Astarion is trying to grab Durge's leg and get her attention, but she's oblivious to it (probably having a brain lapse moment).
Auntie Ethel automatically started talking to Gale during the fight, and then Mayrina automatically talked to Gale rather than to my main character - resulting in her and Astarion showing up in the background.
